Amputation Bed:  
Amputation Bed: Amputation is the cutting or removal of body parts by surgery to prevent spread of infection or gangrene or some cancers. Common parts are usually limbs both upper and lower limbs.

Causes of amputation are diabetic foot, extensive infection with necrosis and dead tissue as in road traffic accidents. Amputation bed is prepared to provide maximum comfort to patient. It is made in the manner to easily handle the patients by medical staff and family.

The things which are required to make amputation bed are:

1.    Normal bed with mattress and plastic sheet.
2.    Two or more blankets
3.    Tow or more pillows
4.    Sand bag
5.    Tourniquet which is plastic tubing to constrict the part in order to prevent bleeding
6.    Instruments for care of amputated parts.

A nurse should follow the instruction of doctor to  prepare bed for this type of patient because amputation of different parts require angulations of bed to different  degrees, for example in case of lower limb amputation, lower side of  bed is kept elevated or a pillow should be kept  under the amputated limb to avoid bleeding.

Bed is prepared in usual manner and a plastic sheet is placed beneath the amputated part and a blanket from above covers it.

Bed Making:  
Bed Making: In nursing, bed making is an important step. It has great importance for patient because patient suffering from different diseases require specific types of beds in hospital as well as at home. While preparing bed for patient nurse should take care of the following points:

1. When nurse makes bed for patient he or she should take care that it must be done without touching clothes of the nurse with the bed sheet.
2. The bed sheet along with mattress should be changed without touching the floor and if solid object is found on bedding, it should be thrown in dustbin.
3. Bed sheet should be folded and pressed against the corners of the mattress so that there are no wrinkles or folds on the surface of bed sheet.
4. Handle bedding or bed sheet with the tips of fingers not with the nails.
5. Alighn all the beds in ward properly in a row and there should be sufficient space between the beds so that patients can mobilize easily when required.
6. It is important to change bed sheets daily or hourly depending on condition of patient. for example if patient is having any incontinence of urine or stool, bed sheet may be soaked or spoiled by the secretions so it has to be changed frequently.
7. After some period bed along with sheets should be kept in open sunlight to kill germs by the natural ultraviolet light of sun.
8. When bedding process is completed then clean sterile pillow is kept on head side of bed. Also keep a blanket at the other end and fold the corners of blanket under the mattress so that patient can use it whenever required.
9. Used bed sheet should be kept in a separate stool or bench and should not be thrown on floor.
10. When changing bed sheet lift sheet from the corners and fold them towards the center and lift the bed sheet from center of the bad so that any germs present on upper side of the sheet do not reach mattress. If any waterproof plastic sheet is present on bed, change it in the same manner.

Cardiac Bed :  
Cardiac Bed: Cardiac bed is made with special arrangements, which are required by a cardiac patient. Cardiac patientís bed is made in a manner to ease the respiration of patient.  Bed is provided with extra pillows to be kept on head side of patient to keep the patient in prop up position for better airflow.

There is special cardiac table provided with the patientís bed with all equipment available for emergency cardiovascular support, like oxygen masks, nasogastric tubes etc. Special care has to be taken for sterilization of bed sheets and clothing because patient is already in critical position and any superimposed infection can be lethal for the patient.

Bed should have proper wheels in working position and nurse has to check the general condition of bed and keep it in ready position to shift the patient to intensive care unit or Operation Theater in case of emergency. A small table for meal should be part of bed to make possible for the patient to take meals easily.

Nurse should provide the bell or cordless phone to family and patient to call medical staff and doctors in case of worsening of patentís condition. Oxygen cylinder should be ready and kept near the bed or ward to provide oxygen immediately to patient   and a nurse time to time to prevent any delay in oxygen therapy should check oxygen flow meter for competency.

Sores or ulcers should be cleaned daily with antiseptic solutions and if needed necroses tissue should be removed by OT staff. Dressing of pressure sores should be done daily and they should be kept dry.

Cystoscopy:

Ecocardio Graphy: Echocardiography is simply defined as ultrasound of heart. It is shortly abbreviated as ECHO in medical terminology.

Electro Cardio Gram: Most of the people are aware of what is ECG. ECG is short term for electrocardiography. ECG is performed right away without any prior preparation or anesthesia. Person should be calmed and counseled before ECG because stress and anxiety can give abnormal ECG patterns.

ECG is taken at rest and during exercises to detect any changes during exertion. ECG taken during exercise is called stress test or stress ECG, or exercise tolerance test. ECG, which is continuously taken for 24 hours period, is called Holterís monitoring. After age 40, ECG should also be done for screening any heart disease in normal persons especially with family history of heart disease, high blood pressure and in obese people, because prevention is better then cure.

Nasal tumors and cancers are rare causes of nose bleeding, severe blow and blunt trauma to head or nose can also result in nasal bleeding. Top most systemic cause is high blood pressure, next comes coagulation disorders, anemia, liver disorders, viral infections, and snakebite and some drugs can also cause nose bleeding.

There are different diagnostic approaches to diagnose the cause of epistaxis, minor bleeding can be evaluated by history and examination by an ENT specialist (ENT specialist is doctor which is specialized only in ear nose and throat diseases) x-rays of nose and Para nasal sinuses also are helpful, blood tests are essential to diagnose blood disorders, severe nasal bleeding need exploration in operation theater under general anesthesia.

Treatment of nose bleed is pinching the nose at the tip, external application of ice, topical application of antibiotic and vasoconstrictor drugs.

Moderate nasal bleeding requires nasal packing which is done by doctor patient is kept in observation during this period to  observe any further bleeding, if bleeding is not controlled even by these  actions then cautery is done (cautery is freezing the bleeding point by electrical probe or chemicals).

Fracture Bed: As its name implies, fracture bed is specially designed for patients with bone fractures and dislocations. It is usually orthopedic bed with overhead frame for traction apparatus.

Traction is a term, which is defined as pulling or giving pressure. Usually fractured parts are supported with slings and woody stripes to prevent mobility to give time to bone healing.

These frames are designed to support fractured parts. In addition fracture bed is made firm by fracture board which is a plain board kept under the mattress to ease the mobility of patient. Because patients having fractures need prolonged rest and immobility, nurse should provide water bags or sand bags to prevent pressure sores. 

Bedpan should be provided near the bed or if patient with upper limb fracture can walk, bed should be near to washroom and toilet. Bones are very fragile and in broken state, they easily acquire infection from clothing and other objects so strict sterilization of bed sheets and other objects near the bed is very necessary otherwise infections can be lethal.

Nurse should provide small dining table over the bed to serve meals. Extra pillows should be given to support different parts of body.

Symptoms Of Haemorrhage:  
Symptoms Of Haemorrhage: It is the medical terminology for bleeding from any site. Hemorrhage are of two major types; external and internal, external hemorrhage is one which is evident  externally, not hidden, internal hemorrhage is one which is hidden inside the body cavities, or in spaces or tissues, hemorrhage can be minor or major, major hemorrhage is the threat to life if not controlled immediately.

Symptoms of hemorrhage are blood loss from any site of body plus, pale cold clammy skin, increased heart rate, shortness of breath and respiratory difficulty, hemorrhage or bleeding in brain tissue rapidly results in altered level of consciousness, confusion, seizures, and paralysis of body depending on the region of brain involved, collection of blood in tissue due to internal bleeding is called hematoma.

Hematoma is a space occupying lesion and cause pressure effects on adjoining structures like hematoma in pelvic region after delivery of child can result in constipation, pain and urinary obstruction in females, hematoma can also be infected resulting in abscess formation diagnosis is external hemorrhage is obvious  and easy to made, internal bleeding if suspected is  a medical emergency and should be confirmed by ultrasound, CT and MRI scans.

Treatment includes securing the bleeding site by pressure bandage, cold, and sutures if tissues and blood vessels are damaged. IV line should be maintained and fluid is given to replace fluid loss until blood is arranged, whole blood or blood products should be arranged and transfused immediately.

Internal bleeding is managed by surgical exploration and drainage of hematoma and ligation of vessels is required to maintain homeostasis.

Unoccupied Bed:

1. Iron bed with spring and wheels.
2. A good quality mattress.
3. A bed sheet preferably white in color.
4. Neat and clean blanket.
5. One or two pillows.
6. Plastic sheet.

Keep the bed on the desired location and keep all the above-mentioned stuff on some neat and clean bench. Do not keep the things on floor. Then spread mattress on bed. After that place bed sheet on the mattress and fold its corners inside the mattress.

Then spread waterproof plastic sheet over the bed sheet. It is not required for all the patients but used in immobile patients who cannot use washrooms repeatedly. Keep blanket on lower 1/3rd of the bed and also fold its corners under the bed sheet, keep pillow on the other side.
